Default to recognition over recall, and put the most frequent action where the hand already is. Clinical staff work in interruptions, so the interface has to survive being abandoned halfway through: preserve state, make it obvious where someone left off, and never discard entered data on navigation. Density is usually correct in clinical tools, because scrolling costs more time than a compact layout costs comprehension, provided the hierarchy and grouping are genuinely clear.

Alert fatigue. When everything is marked urgent, nothing reads as urgent, and clinicians learn to dismiss warnings reflexively, including the ones that matter. The design fix is a real severity hierarchy with very few things at the top, interruptive treatment reserved for the small set that genuinely warrants stopping someone, and a visible reason attached to every alert so the user can judge it rather than simply clear it.

Prefer undo over confirm wherever the data model allows it, because confirmation dialogs stop being read the moment they become routine.
